Before Your First Visit
Before your first appointment, please complete the following forms and bring them with you when you visit our office.
This form will cover any personal and family information needed to establish your file and discusses your current eye health and vision status. Your responses help guide the responses of your optometrist and the rest of our optometry staff and remind us to address any significant issues or concerns you may have.
Many general health concerns and conditions can affect your eye health and vision. That is why state health boards and almost all medical and optical insurance plans require this information This holistic approach to healthcare also allows your optometric team to treat you as a whole person, not just a pair of eyes.
On your form, please include a complete list of all prescription and non-prescription medications you are currently taking. If it makes you more comfortable, you may list your medications on a separate list for us to photocopy if you prefer.
Please Bring the Following to Your Appointment
To help ensure your first visit with us goes smoothly, please bring:
Your insurance card, government-issued photo ID, and any relevant insurance claim forms.
If an eye health or vision issue is discovered during your exam, it helps to have the relevant insurance claims form on hand. This helps streamline your insurance claims process.
Any glasses or contact lenses you currently wear.
Contact lens wearers should wear their contact lenses and bring their contact lens case, as well as their carton or lens packet. This lets your optometrist easily identify your contact lenses lens series, power, and manufacturer.
Any eye drops or ointments you are currently using.
Please place these in a small bag and bring them with you to your appointment. Your optometrist will review these products, and if necessary, suggest more appropriate alternatives.
Dr. Gregory Foley has been practicing optometry for over 25 years, with a career spanning from North Carolina to the D.C. metro area. He holds a Bachelor of Science in Biology, a Bachelor of Science in Optics, and a Doctor of Optometry. Since starting his profession in 1993, he has worked with top specialists in cataract, glaucoma, pediatric, retina, and oculoplastic fields. During this time, he was granted hospital privileges at NorthEast Hospital in Concord, NC, where he developed an interest in non-surgical ocular disease and trauma management.
In 2007, he moved to the D.C. metro area to continue his work at Eye Central, where he was made a partner in 2009. After the merger with Associates in Eyecare in 2017, Dr. Foley started acting as managing partner for the Eye Central Offices under the AIE umbrella.
Dr. Foley is quite prolific in many aspects of his field, from giving educational lectures to publishing five articles in the “Review of Optometric Business”.
Outside the office, Dr. Foley is an avid triathlete and has competed in numerous events over the years. Since moving to the D.C. area, he has become involved with a master’s level swim program and competes with a multi-sport race team for Bike Rack.
Dr. David A. Fissel has experienced a varied career thanks to his educational experience and his willingness to serve the optometry community. He graduated first in his class from the University of Pennsylvania and second in his class at The Pennsylvania College of Optometry. During his education, he received numerous clinical and scholastic awards.
Dr. Fissel stays active in the optometry community by serving as the chairperson for the Board of Optometry Appointed by the Mayor of D.C., the past-president of the Board of Optometry, and the past-secretary of Optometric Society of the District of Columbia. He has also served as a board member for the Virginia Optometric Association, American Optometric Association, and the Optometric Council for the National Capital Region.
In his spare time, he loves to travel and stay fit.
Dr. David A. Reed has an extensive career in optometry spanning over 42 years. This experience has made him a wealth of knowledge for his peers, especially in the field of contact lens fittings.
His career has included time spent as the president of the Virginia Optometric Association, past-president of the North Virginia Optometric Society, and chairman for the Contact Lens Committee of Virginia Optometric Association. He also served as a board member for various associations, including Capital Eye Consultants, Vision Care Services in Washington D.C., and the Advisory board of Bausch & Lomb Soflens Division.
Since dropping down to 2-3 day workweek, Dr. Reed likes to spend his free time reading, traveling, cooking, and hitting the gym.
Dr. Ann L. Martin earned her Doctor of Optometry from the Indiana University of Optometry, graduating top of her class while being a member of the distinguished Beta Sigma Kappa Honor Society for all four years of her education.
Her career has led to work in a variety of optometry practices including some in Champaign, I.L., Omaha, N.E., and Lincoln, N.E. Her experience in contact lens fittings, pediatrics, ocular disease management, and co-management of cataract and refractive eye surgeries led her to develop a love of vision care, giving her patients the best experience she can give.
In her free time, she loves spending time traveling, practicing yoga, crafting, and spending time with her twin boys.
Dr. Cameron N. Jones graduated with honors Nova Southeastern University College of Optometry in 2014 while serving as a member of the optometric society Beta Sigma Kappa. After graduation, she moved to Chicago, I.L., to pursue a combined residency at Jesse Brown VA Medical Center in ocular disease and low vision.
After her residency, she completed the Advanced Competency in Medical Optometry exam and has been practicing primary eye care in the D.C. metro area since 2015.
In her free time, she loves to hike, bike, run, play pickleball and spend time with her family.
Dr. Tufail is a native Houstonian, where she graduated Magna Cum Laude from the University of Houston with a Bachelors in Psychology, minor in Biology. She then graduated from University of Houston College of Optometry where she earned her Doctorate of Optometry degree.
After graduating, Dr. Tufail worked at a private practice offering comprehensive eye exams with a heavy focus on speciality contact lenses, dry eye disease management, and co-management of refractive surgery. She has extensive training in primary care with a particular interest in dry eye disease and specialty contact lenses. She is certified in orthokeratology by the manufacturers and designers of several orthokeratology lenses.
Dr. Tufail recently moved to Northern Virginia, where she resides with her husband. In her spare time, she enjoys outdoor activities, traveling, eating dessert, and spending quality time with her loved ones.

Environmental Initiatives
At Eye Central, we are continually striving to be more environmentally friendly. To help us achieve this goal, we launched our electronic medical record system in 2012 to reduce paperwork.
We have also increased our recycling efforts. We will gladly accept your #5 plastics (which are used to manufacture disposable contact lens blister packs) as part of Preserve Product’s GIMME 5 program. We are also proud to carry Eco Optics designer frames, which are made from 95% recycled material.
